Case Management
Purpose
The primary focus of the case management program is to provide older adults support, resources, and community services. Through in-home visits and phone calls, our case managers communicate with older adults and their families to help them cope with the stresses of deteriorating health, the anxiety of being alone, and the difficulties of managing everyday life. We help older adults remain in their homes, feel connected, safe, and healthy. We offer consultation and advice, whatever the need, we are here to help. Some individuals are looking for short-term information, while others benefit from ongoing case management support.
What We Can Provide
Our case managers are trained and caring professionals who are knowledgeable about issues related to aging and community resources available for older adults. We can provide information, referral and assistance in the following areas:
Case Management assistance is available to individuals 60 and over who live in one of the following areas: Town of Dane, Town of Springfield, Town of Westport, Town of Vienna, Village of Dane and Village of Waunakee. We are available for appointments at the Waunakee Senior Center or in your home. For more information, please contact case managers Candice Duffek or Melissa Woznick by calling 608-849-8385.
Confidentiality
Referrals to case managers remain confidential. Case managers only share information with other provider agencies when necessary for the purpose of providing services.

- Personal Emergency Response Systems (Lifeline)
- Prescription Drug Coverage
- Transportation Options
- Turning 65/Retirement
